By signing below, I acknowledge that this youth flag-football skills training camp is hosted and presented by Elite Sirius Sports, Coach Aulden Knight II (Head Coach of the University of Michigan Women’s Flag Football Team), and Meraki Sports TX. I understand that flag football and athletic training involve inherent risks, including falls, collisions, sprains, fractures, head injuries or concussions, heat-related illness, and other foreseeable or unforeseen injuries.
I understand that participation is voluntary and that the participant assumes the inherent risks of participation. I confirm that the participant is physically able to participate, or I accept responsibility for determining whether the participant is fit to participate. If the parent/guardian cannot be reached in an emergency, I authorize event staff to obtain emergency medical treatment if needed. I understand that the participant and/or parent/guardian is responsible for medical costs not otherwise covered.
To the fullest extent permitted by law, I release and hold harmless Elite Sirius Sports, Coach Aulden Knight II, Meraki Sports TX, and their staff, volunteers, coaches, agents, and facility partners from claims arising out of participation, except to the extent caused by gross negligence or intentional misconduct. I agree that organizers may remove a participant for unsafe or disruptive behavior. I confirm that I have read and understood this waiver before signing.

Privacy Notice
This form is intended to be completed by a parent or legal guardian for a minor participant. Children should not submit this form on their own.
Information we collect
Elite Sirius Sports collects the participant and parent/guardian contact information, school and grade information, emergency contacts, medical conditions or accommodations, optional insurance information, payment-method preference, consent choices, electronic signature, submission date and time, browser information, and internet address associated with the submission.
How the information is used
The information is used only to register and communicate with participants, organize the camp, assign training groups or time slots, prepare shirts, support participant safety and accommodations, respond to emergencies, document consent and waiver acceptance, maintain event records, and address legal or insurance-related matters.
Storage and access
Completed submissions are stored in the secured database connected to this registration website. The blank form is public, but completed registrations are not displayed publicly or added to a public ChatGPT conversation. Access is limited to authorized Elite Sirius Sports personnel and service providers that host or operate the form. Information may also be disclosed when reasonably necessary for emergency treatment, safety, legal compliance, or the protection of participants and organizers.
Sharing, sale, and retention
Elite Sirius Sports does not sell participant information. Information is retained only as long as reasonably necessary for camp administration, safety, recordkeeping, insurance, dispute resolution, and applicable legal obligations, after which it should be securely deleted or de-identified.
Parent and guardian choices
A parent or legal guardian may request access to, correction of, or deletion of submitted information, subject to legal, safety, insurance, and recordkeeping requirements.
